**These forms are Word templates, which
means they need special handling. Click on the link to get a form
(it may take a minute or so), then use the File menu to Save it
in the folder where your Word Templates are stored. By default
this will be
C:\Program
Files\Microsoft Office\Templates
but if in doubt, search for a .dot file (all templates have a
".dot" extension).
Once you have saved it, you can access it in Word by selecting File -> New which will give you access to all your templates. Choose one and open it. You can then fill in the various boxes and save the result as a normal Word document - this will not change the original template.
If you wish to make permanent changes to the template, eg to put your own name in the appropriate places, open it as above, make the changes and then select File -> Save as ... In the dialogue box that pops up, choose Document Template as the type, and save it under a new name.
